Please read carefully the ‘Notes on Rockschool Music Examinations’ before completing the entry form.
請於填寫報名表前仔細閱讀「Rockschool考試注意事項」及報名須知。所有已遞交之申請，本公司均視為了解、同意有關條文並嚴格遵守。如有爭議，本公司保留最終決定權。

Select the code number (01-11) of the instrument for each candidate

E.Drums Model: Roland TD17KL, Roland TD30KS   電鼓型號: Roland TD17KL, Roland TD30KS
Aside from acoustic drums(04). electronic drums(05), vocal(06), piano(07) and classical piano(11) examination, all candidates must bring their own instruments for their examination.
除了樂器編號 04，05，06，07，11 以外，所有考生必須自備樂器出席考試。

Examination Period 考試日期: 2025 Period B October - November (2025年第二期考試10至11月)

						
Entry Information 報名須知
1.	Complete submission must include:
i.	the completed entry form;
ii.	upload a photocopy of candidate’s Identity Card/ Passport (with photo); 
AND 
Entry will NOT be accepted if any of the requested attachments have not been submitted. 
(photocopy of the pay-in slip for own record is recommended)
  
申請考試人士必須將下列文件一併交回：
i.	已填妥之報名表；
ii.	上傳考生的香港身份證(附有照片)／護照(附有照片)副本；及
  有關文件如未有連同報名表一併遞交，申請恕不獲接納。
 
2.	Payment of examination fees must be online payment;
*Candidates are responsible for any additional charges required by the bank

考試費用必須以網上付款方式繳付：
*如因銀行或其他付款方式所收取之手續費，本機構恕不負責，一切額外費用，考生需自行承擔。

3.	Full payment of examination fees must be paid on or before the closing date of the entry period of the corresponding examination.  Supplementary fee will be charged for any discrepancy in payment. Examination fees are NOT refundable.
全部考試費用必須在該考試報名截止日前繳付。如申請人所繳付之費用與申請該考試所需繳付的全部費用不相符，本公司有權向申請人額外徵收行政費用。任何已繳交之報名及其他費用，將不獲退還。

4.	The exam fee includes exam enrollment administrative fee as well as the mailing fees of confirmation letter (if applicable), exam marksheet and certificate (if applicable). If the mail is returned due to delivery problems, candidate needs to collect the mail at Rockschool office in person. Please note that Rockschool HK will NOT resend the mail by post.
考試費用已包含考試報名的行政費，確認信(如有)、考試分紙及證書（如有）的郵寄費用。考試文件若於首次寄出後，因郵遞問題退回本公司，考生需親自到本公司領取該文件。本公司不會重新郵寄該文件。

5.	Examinations are processed in English. For candidates who need an interpreter (English to Cantonese only), an additional fee of HK$180 is required, and must be paid together with the examination fee.  
考試以英語進行，考生如需要傳譯服務 (只限英語傳譯廣東話)，請於報名表上列明，每位考生需另付傳譯服務費港幣180元正，此費用必須與考試費一併繳付。

6.	Instrument, type of exam (Grade or Performance) and grade of submitted applications cannot be changed. HK$200 administrative fee charged for any amendment in exam details made after submitting the entry form.
遞交後的申請均不可更改科目、形式或等級。遞交報名表後，本公司有權對一切考試相關之改動徵收行政費港幣200元正。
 
7.	For drummers, if you need to add any additional hardware for exams, you must give prior notice to Rockschool HK before the release of exam appointment slips.
流行鼓考生若需攜帶額外配件進行考試，必須於准考證發出前通知本公司。

8.	The Rockschool HK Ltd. Will not be responsible for any delay/loss in mail or internet service.
任何因郵遞延誤或遺失及系統故障導致有關文件未能如期寄到收件者地點，本公司恕不負責。

9.	Any candidates who failed to present valid appointment letter and identification document with photo of the candidate may be refused to sit in the exam. If candidates are permitted to take an exam, candidate must* bring back the documents to the exam steward for verification.
*Exam in the morning must bring back in the afternoon of the same day before all exams end; exam in the afternoon must bring back on the next day before noon.
如考生於考試當日未能出示有效准考證及附有考生相片之身份證明文件，試場助理有權拒絕有關人仕在未能提供所需文件前進行考試。即使考生獲批准入內考試，考生必須* 親身帶回以上文件供試場助理核對。
*上午之考生必須於當天所有考試完結前; 下午之考生必須於翌日中午前。

10.	Photocopying of any material to facilitate page turns in the exams is allowed. Original exam book, or electronic book must be brought to the exam. Re-used book is not accepted in the exam except retake.
任何影印之考試材料只可作翻頁用途。考生必須帶備原裝考試書本或電子版考試書本應考。除重考生，考試書本恕不可重用。
Candidates who failed to fulfill the above request will be disqualified.
如考生未能達到以上要求將會被取消考試資格。
 
 
Personal Data 個人資料
 
1.	Personal data given must be clearly written, true and accurate.
填寫之個人資料必須清晰易見，並且全屬確實無訛。

2.	Personal data given will be used for examination purposes ONLY.
填寫之個人資料只作辦理考試安排之用。
 

General Information 一般事項
1.	Confirmation letter and Exam appointment slips will be sent by emails to the entrants in mid-Sept. Please contact Rockschool HK Ltd at tel: 27377501,email:info@rockschool.org.hk if the exam appointment slips have not received on 17th September 2025.
 確認信及准考證將於9月中以電郵發送至申請人，申請人如在2025年9月17日仍未收到准考證，請與本公司聯絡 (電話: 27377501; 電郵: info@rockschool.org.hk)。

2.	Rockschool HK Ltd. reserves the right to change the examination dates and entry dates listed above.
本公司(Rockschool HK Ltd.)保留更改以上考試日期及報名日期之權利。

3.	Entry must be made on or before the closing date of the entry period.  Supplementary fee of HK$200 will be charged for ALL late entries (late entries will ONLY be accepted within 7 days after closure of entry period).
考試申請必需在報名截止日期前辦妥，本公司會對逾期報名徵收附加費用港幣200元正(本公司只接受報名截止日期後7天內之考試申請)。

4.	Candidates aged below 18 must be entered by an entrant aged 18 or above.
未滿18 歲之考生，必須由年滿 18 歲之人士代為申請考試。

5.	Candidate name(s) given on the entry form must be agreed with the candidate(s) attending the examination. Examination entries may not be transferred from one candidate to another under no circumstance. Rockschool HK Ltd examiners and exam operator have the right to verify candidates’ identities.
報名表上之考生姓名，必須與出席考試之考生姓名相符，任何情況下考生不能被轉換。本公司之考官及試場助理有權對考生之身份進行核實。

6.	Supplementary fee will be charged for any rescheduling or deferral applications made after the appointment slip has been issued. All rescheduling or deferral applications process once only.
i.	Rescheduling application within the same exam period incurs a supplementary fee of HK$200. The application should be submitted within 5 working days after the appointment slip has been issued. Application after the deadline will not be accepted.
ii.	Deferring the exam to the next period incurs a deferral fee of half the original exam fee. Deferral application must be submitted with supporting documents within 10 working days after the appointment slip has been issued. The exam can only be deferring to the next exam period. Applications after the deadline will not be accepted. Fees of unsuccessful applications are not refunable.
Changing an examination date when it has been confirmed is very difficult, Rockschool will try processing but this cannot be guaranteed, with final decision reserved.
*Please note that the above is not applicable to Music Theory Exam.
准考證發出後，欲申請更改任何已落實的考試日期，本公司會徵收行政費。所有改期申請，不論成功與否，只會調動一次。
i.	如申請同一考試期內改期，書面申請必須在准考證發出後5個工作天內遞交（以郵戳或電郵日期為準），本公司會徵收行政費港幣200元正。逾期申請將不獲處理。 
ii.	申請延遲任何已落實的考試日期到下一個考試期，書面申請必須在准考證發出後10個工作天內遞交（以郵戳或電郵日期為準），其附加費為該考試費用之一半，不成功申請的費用將不獲退款。請注意，考試只可以延遲到下一個考試期，有關申請必須附上有效證明文件，如未能提供，本公司有權拒絕申請。 

本公司必須明確釐清，有關行政費只表示本公司會處理有關申請，在任何時候也不表示必定申請成功。更改任何已經落實的考試日期是非常困難的，本公司會盡量應申請作出安排，在任何時候均不保證申請必定成功辦理。所有爭議，本公司保留最終決定權。


7.	Examinations will take place at various examination centres in Hong Kong for all entries processed through Rockschool HK Ltd.
所有經本公司遞交之考試申請，其考試將會安排於香港不同區域之考試中心內進行。考生不能要求在指定試場應考。
